壹、活動目的：為了解師生對各餐廳的滿意程度，藉此作為廠商績效評鑑（滿意度評分佔35%）及總務處續約與否之重要參考，並選出餐廳「優良服務的攤商」，以為楷模，藉以督促業者提升服務品質。

統一超商禮券1000元5名、500元10名、200元50名，以隨機方式抽出，並在2021年12月30日於學務處首頁/最新消息，公布幸運得獎名單。(本籍得獎者預計發放虛擬商品卡，將以簡訊發送至填寫問券時留下之手機號碼，憑條碼至超商進行消費；非本籍者須預先繳稅後再發放實體禮券)

2021 Canteen Satisfaction Survey Questionnaire

[I. Purposes of this survey]:

To gain better understanding of the satisfaction rating and other related feedback of the teacher and student body concerning the dining establishments in the campus. The survey results will provide us with ideas to better serve you.

[II. Duration of this survey]:

Dec. 23 - Dec. 28 , 2021

[III. Way of conducting this survey]:

Evaluation questionnaires are to be completed online by all students and faculty.

[IV. Way of rewarding]:

To encourage all teachers and students to fill out the questionnires online, we provide the following rewards: five $1000 gift voucher of the 7-11 mart convenience store, ten $500 gift vouchers of the 7-11 mart convenience store, and fifty $200 gift vouchers of the 7-11 mart convenience store. The voucher winners will be drawn ramdomly and the raffle result will be announced on the websites of the Office of Student Affair in Dec. 30th 2021.

    If the person who is non-resident of the Republic of China, regardless of the amount of the winner, the tax will be withheld for the winning income.

    The rate of “Income from Contests and Games and from Prizes and Awards Won by Chance” is 20%.

    If the winner refuses to pay the tax, the winner is deemed to have given up the prize.
