一、活動目的：

為了解師生對各餐廳的滿意程度，藉此作為廠商績效評鑑（滿意度評分佔35%，辦法公告於學務處/生輔組/服務資訊/膳食輔導網頁）及總務處續約與否之重要參考，並選出餐廳「優良服務的攤商」，藉以鼓勵業者提升服務品質。

I. Purposes of this survey:

To gain better understanding of the satisfaction rating and other related feedback of all students and faculty concerning the dining establishments in the campus. The survey results will be used as a reference for service improvement.

二、活動時間：114年12月22日起至12月28日止。

II. Duration of this survey: Dec. 22th - Dec. 28th, 2025

三、活動方式：由全校師生線上評選。

III. Way of conducting this survey:

Evaluation questionnaires are to be completed online by all students and faculty.

四、抽獎活動：

為鼓勵全校師生上網評選，本次活動提供的獎品為萊爾富商品卡（1000元1名、500元2名、200元40名），以隨機方式抽出，並在115年1月上旬於學務處首頁/最新消息，公布幸運得獎名單。

IV. Way of rewarding:

To encourage all teachers and students to fill out the questionnires online, we provide the following rewards: one $1000 gift voucher of the HI-LIFE mart convenience store, two $500 gift vouchers of the HI-LIFE mart convenience store, and forty $200 gift vouchers of the HI-LIFE mart convenience store. The voucher winners will be drawn ramdomly and the raffle result will be announced on the websites of the Office of Student Affair in early January 2026.
